She definitely says would. This still sounds a little strange to me to be honest, and I am not certain of what her meaning is.. She (your girlfriend) just left you here by yourself? If I were her, I would kill you instead (of leaving you here).. Maybe she knows his nature, that he might embarrass his girlfriend, and is exaggerating about what she would do in that situation if she were the girlfriend? (Killing him would be better than leaving him by himself with other people) But then why not just take him with you to the hospital? It's a stretch... I am just as confused as you are about this dialogue. It sounds unnatural to me even if she is a native speaker, and you were right to feel that it is random (I think so too).

In this conversation,it can be a friend making jokes,his girlfriend left him alone,and he could be a murderer.So it's basically just joking with friends.

Are you sure A didn't say "I could kill you" ? "I would kill you" doesn't make sense to me in this situation.

나 같으면 널 여기서 버리고 가지 않고 죽일 것이다 Something like this.. but I think it's weird even in English.

It is pretty extreme but the closest thing I can think of that is similar in Korean is '배고파 죽겠어' it's not that the person is going to die at that moment if they don't eat it's an exaggeration to show how they feel, the same with this sentence. It's to show how the person would not have let that happen if it were them, no matter what. If we complete the sentence it would be more like "I would kill you if you did the same to me"
