(Killing him would be better than leaving him by himself with other people) But then why not just take him with you to the hospital?

It is pretty extreme but the closest thing I can think of that is similar in Korean is '배고파 죽겠어' it's not that the person is going to die at that moment if they don't eat it's an exaggeration to show how they feel, the same with this sentence. It's to show how the person would not have let that happen if it were them, no matter what. If we complete the sentence it would be more like "I would kill you if you did the same to me"
